  • Passport

If your child is under 18, a parent or legal guardian must be present, and a birth certificate and school ID must be provided. If the legal guardian does not share the same last name as the minor, required paperwork like a birth certificate and proof of legal guardianship must be shown. For earlobe piercings, the child must be at least five years of age and have the necessary documentation.
